BEFORE YOU PROCEED

You will need 2 spoons to hold your cheeks out of the way. It’s also recommended that you have someone help you take photos. Be sure to use flash on your camera to help light your teeth and mouth. The next few images are examples of how to take each photo. You will be able to upload all of your photos at the end of the form.

*IMPORTANT*

If you are in aligners (invisalign® or Island Smile Aligners) you MUST upload 2 versions of each photo. One photo with your aligners IN (and elastics, if applicable) and one photo with your aligners OUT.
